• Follows the CPOC or Assignment established by the Manager or designee.
  
• Assist patient with activities of daily living as outlined on assignment.
  
• Assist with patient/client transfers.
  
• Change and/or launder bed linens.
  
• Feed or assist with meals.
  
• Maintain a clean and safe environment.
  
• Light housekeeping.
  
• Documentation of patient/client care or services provided.

• Direct support staff shall be at least 18 years of age.
  
• Shall possess a high school diploma, or a GED, or a trade school diploma in the area of human services.
  
• Demonstrated competency, or have verifiable work experience in providing support to individual with disabilities.

• 1 year of current experience and current CPR.
